ActiveRx appreciates how busy you are so we’ve created a simple 3 step, online process for you to verify your insurance coverage, complete your new patient forms, and schedule your first appointment. The entire process takes less than 3-minutes online and will save you up to 30 minutes by eliminating the need for a phone call and early arrival to your initial appointment.

Will insurance pay for my care at ActiveRx?
In most cases, your health, auto or worker’s compensation insurance will cover the cost of the care you receive. If you submit your insurance information to us electronically via the new patients button above, one of our staff members will contact you to review your benefits and schedule your first appointment. If not, we will confirm your benefits on the day of your first appointment. While verifying your benefits is a service that we provide for you, we always recommend that you contact your insurance provider personally to confirm your physical therapy benefits as well.
